Utilize ISMA’s SDOH resource toolbox

To make Indiana a healthier state, we must address social drivers of health (SDOH): the nonmedical factors influencing health outcomes.

The ISMA launched an online toolbox in September, “Drive for Five,” and a webpage to assist physicians and their patients in accessing resources concerning SDOH. The name Drive for Five comes from the five SDOH domains from Healthy People 2030: health care access and quality; neighborhood and built environment; social and community context; economic stability; and education access and quality.

Drive for Five is a one-stop SDOH resource page for physicians and their practices. Visit www.ismanet.org/driveforfive to gain free, instant access to the platform.  It can be viewed on your mobile phone, tablet, or desktop by clicking “add” to download it on your home screen for convenience. Resources include Indiana 211, Mental Health and Substance Use Disorder facility locators, provider-to-provider consultation, screening questionnaire templates, and billing and coding information for physician reimbursement.    

A committee of ISMA-member physicians has vetted all Drive for Five resources. ISMA announced the start of the multifaceted SDOH initiative in April.
