The Centers for Medicare and Medicaid Services' proposed Medicare Physician Fee Schedule for calendar year 2027 is released in July. CMS traditionally releases the proposed rule in July, launching a public comment period and setting the stage for policy changes that will take effect the following year. The proposal is expected to address physician reimbursement, quality reporting requirements, value-based care programs and telehealth policies.
The annual physician fee schedule remains one of the most closely watched federal actions for ISMA because it directly affects Medicare reimbursement and practice operations. Recent proposals have included payment updates, modifications to alternative payment models and revisions to quality programs. Physician organizations, including the AMA, are expected to closely analyze the proposal and advocate for changes before a final rule is issued later this year. ISMA continues to advocate for a more stable payment system that reflects inflationary pressures and rising practice costs. Federal lawmakers have increasingly discussed long-term reforms as concerns grow about the sustainability of physician practices and patient access to care.
Congress also is expected to focus on fiscal year 2027 appropriations legislation before lawmakers leave Washington for the August recess. Funding decisions under consideration could affect agencies and programs important to physicians, including the National Institutes of Health, the Centers for Disease Control and Prevention, the Health Resources and Services Administration, and graduate medical education programs.
Several congressional committees with healthcare jurisdiction are expected to continue examining issues such as Medicare payment reform, physician workforce shortages, Medicaid financing, artificial intelligence in healthcare and rural healthcare access during the summer work period.
The various decisions and proposals released this month are expected to shape the federal healthcare agenda for the remainder of 2026 and into 2027.
