The Indiana Department of Health Infectious Disease Epidemiology & Prevention Division is conducting a brief survey, which will take less than 10 minutes, related to pertussis in Indiana. Voluntary participation helps identify what necessary resources are needed to distribute to prevent and control the spread of this disease in Indiana. All data is anonymous and used for research purposes only.
