The Indiana State Department of Health (ISDH) is asking physicians and other health care workers who may be interested in volunteering to provide relief to those on the front lines of this pandemic to complete the survey at the link below.

The ISDH commends the cooperation and dedication of health care workers during the COVID-19 pandemic. The agency understands the stress and physical demands that are being placed on our health care system would like to alleviate it as much as possible.
