Program description:

The rate of recidivism for incarcerated individuals in the United States is 70% within five years of release, with few resources to assist re-entering individuals in finding housing, gaining employment or accessing social services. ISMA has resolved to focus attention on efforts that remove barriers and increase access to social services and benefits for re-entering individuals, such as food subsidies, health care and housing. ISMA is also committed to educating physicians about ways to connect re-entering individuals with needed resources in their communities.

At the end of this webinar, participants will be able to:

  • Identify the medical, behavioral health, and social challenges that individuals face when they leave incarceration.
  • Examine how CareSource innovatively serves those leaving the Indiana Department of Correction.
  • Describe how physicians play a critical role in the community ecosystem necessary to meet the complex needs of the returning citizen and foster successful re-entry into society.
