SESSION 1 – INTRODUCTION TO E/M AND THE PATIENT HISTORY
Objectives

  • Introduce E/M documentation requirements
  • Learn why establishing medical necessity in the Chief Complaint is essential
  • Discuss the eight elements in the History of Present Illness (HPI)
  • Identify the appropriate elements needed in Review of Systems (ROS)
  • Recognize elements of Past Family Social History (PFSH)
  • Understand how to put these all together and select the level of History
  • Look at a variety of HPI examples from different specialties 
  • Ask questions about the History Key Component

SESSION 2 – EXAMINATION
Objectives

  • Explore the differences between the 1995 and 1997 E/M Guidelines
  • Discuss specialty exam templates and cautions of EHR recommended codes
  • Become familiar with the pitfalls of “cloned” visits from an auditing perspective
  • Determine the most effective way to support the level of Exam
  • Review a variety of Exam examples from different specialties 
  • Ask questions about the Exam Key Component

SESSION 3 – MEDICAL DECISION MAKING (MDM)
Objectives

  • Distinguish the three different elements of Medical Decision Making
  • Learn how to document each MDM element to withstand an audit
  • Find out how to calculate the level of Medical Decision Making
  • Go over real Medical Decision Making examples
  • Ask questions about the MDM Key Component

SESSION 4 – “PUTTING IT ALL TOGETHER
Objectives

  • Learn how to select the correct level of E/M service in any location
  • Discuss when to bill based on time and what you must document to do so
  • Review important E/M modifiers to ensure appropriate payment
  • Understand “incident-to” billing requirements for different payers
  • Know how to document and bill for “shared/split visits”
  • Recognize why internal audits are essential
  • Ask questions and get answers!
