- Surcharge formula – indexed by specialty; developed by an actuary
- Participating physicians pay to the PCF
- Amount of payout to patient capped at $1.65 million per injury, increasing to $1.8 million on July 1, 2019
- Health care provider or the provider's insurer pays first $400,000, increasing to $500,000 on July 1, 2019
- Paid no later than 60 days after the issuance of a court-approved, final judgment.
