The ISMA group health insurance program renews on July 1 for all physicians with individual policies and many employer groups. Renewal packets were mailed the first week of May.
The following benefit changes will be made on July 1. These revisions bring benefits closer to current market standards while maintaining advantages over competing plans. Increased urgent care and emergency room co-pays will help steer care to the most appropriate and cost effective setting, while continuing to minimize out-of-pocket costs when urgent or emergent care is required. No benefit changes will be made to plans not listed.
The PPO 10000 plan will be discontinued on June 30 because the deductible and out-of-pocket maximums are higher than the minimum standards set by the Affordable Care Act. Subscribers with the PPO 10000 plan will automatically be moved to the PPO 5000 plan on July 1.
What you need to do
To renew your current option, simply pay your premium notice by June 30. To request a change for your entire group (e.g., change medical plans or add dental), email Jolene or fax a note to her at (317) 261-2235.
To request a change for specific employees (e.g., add dependent), fax a completed Anthem Enrollment Application to the ISMA Insurance Department at (317) 261-2235 or send it as an email attachment to Jolene. Print an Enrollment Application from the Health Administration menu here.
To have a form faxed or mailed to you, call the ISMA at (800) 257-4762 and ask for the Insurance Department. Use the same number to call us with your questions; ask for Jolene, Donna or Tom.
