Help patients make decisions about end-of-life care; refer them to an online brochure about POST

Your patients with chronic progressive illnesses, frailty or terminal conditions are likely considering preferences and planning for end- of-life treatment. A Physician Order for Scope of Treatment (POST) form, passed into law by the 2013 Indiana General Assembly, facilitates identification and implementation of patient treatment preferences near the end of life. The document, based on those used in 40 states, is a preferred practice – to improve quality and continuity of care.

The two-page state-recognized POST form should be completed in consultation between patient and physician. A patient brochure explaining this tool is now available on the ISMA website to help patients prepare for this consultation.
