Want to make changes in your ISMA insurance plan? Now’s the time to request them

If you and your staff are insured with the ISMA group health insurance program and your policy renews on July 1, you can now request changes in your medical plans or add dental coverage. This activity is allowed only on the July 1 renewal date each year.

Also on July 1, employees who previously waived coverage can enroll, and insured employees who previously waived coverage for dependents can add them without a special qualifying event. Subscribers may delete dental or delete dependents as well.

All requests for July 1 changes must be postmarked by June 30.

You should have received your renewal packet; all materials were mailed the first week of May. The packets included Plan Options and Rates illustrations, providing rates for current plan selections and all other available plan options.

To renew your current option, simply pay your premium notice by June 30. To request a change for your entire group (i.e., change medical plans or add dental), email Jolene Collins or fax a note to her at (317) 261-2235.

To request a change for specific employees (i.e., add dependent), fax a completed Anthem Enrollment Application to the ISMA Insurance Department at (317) 261-2235, or send it as an email attachment to Jolene. Print an Enrollment Application from the Health Administration menu at the ISMA insurance website.

To have a form faxed or mailed to you, call the ISMA at (800) 257-4762 and ask for the Insurance Department. Use the same number to call us with your questions; ask for Jolene, Donna or Tom.
