The Indiana Professional Licensing Agency (PLA) will email notices reminding you to renew online your medical license, medical residency permit or Controlled Substance Registration (CSR) by June 30.
To renew your license online, simply go here after May 1. Your login ID is your full license number and your password is the last four digits of your Social Security number. However, military personnel being deployed early should contact the PLA office via email.
If you do not have an email account, the PLA will mail a paper renewal notice.
The medical license renewal fee is $200 and $60 for each CSR. If you fail to renew, your medical license will expire at midnight on June 30, and a $50 late fee will be assessed to your license and each CSR that you hold.
Also, the PLA reminds you to make sure the agency has your current mail and email addresses on file.
