You can help families access vaccine records on MyVaxIndiana

With a little help from you, families in your practice can access their immunization records through MyVaxIndiana. The web application is part of the Children and Hoosier’s Immunization Registry Program (CHIRP) and enables parents to view vaccine histories and recommended immunizations for their children from any computer.

However, you must start the process. You need to provide the parents a personal identification number, which grants specific access to a child’s requested record.

The MyVaxIndiana system includes an instruction guide that can be printed or emailed to parents. Read more, or call (888) 227-4439.
