The ISMA group health insurance program renews on July 1 for all physicians with individual policies and many employer groups. In compliance with the Affordable Care Act (ACA), renewal packets mailed in early May include a Summary of Benefits and Coverage (SBC) for each subscriber. The SBC provides standardized and easy-to-understand information about health plan benefits and coverage.
All requests for July 1 changes must be postmarked by June 30.
If your family or practice is insured with the ISMA group health insurance program and your policy renews on July 1, you can now request changes in your medical plans or add dental coverage. This activity is allowed only on the July 1 renewal date each year.
Also on July 1, employees who previously waived coverage can enroll, and insured employees who previously waived coverage for dependents can add them without a special qualifying event. Subscribers may delete dental or remove dependents as well.
Renewal packets were mailed the first week of May, containing Plan Options and Rates illustrations with rates for current plan selections and all other available plan options.
What you need to do
To renew your current option, simply pay your premium notice by June 30. To request a change for your entire group (e.g., change medical plans or add dental), email Jolene or fax a note to her at (317) 261-2235.
To request a change for specific employees (e.g., add dependent), fax a completed Anthem Enrollment Application to the ISMA Insurance Department at (317) 261-2235 or send it as an email attachment. Print an Enrollment Application from the Health Administration menu here.
To have a form faxed or mailed to you, call the ISMA at (800) 257-4762 and ask for the Insurance Department. Use the same number to call us with your questions; ask for Jolene, Donna or Tom.
