Wisconsin Physician Services (WPS), the newly announced Medicare administrative contractor (MAC) taking over for National Government Services (NGS) Aug. 20, is sending you a letter. With the communication will be the required Electronic Funds Transfer (EFT) 588 form, along with instructions.

Set to be mailed in about a week, this letter will be directed to the address on file with Medicare enrollment. You must return the 588 to WPS (address provided in the letter) no later than June 30, 2012. This EFT form gives WPS permission to deposit funds into your bank account when the Jurisdiction 8 (J8) transition happens.

Please be sure to advise the responsible parties at your address to watch for this letter and to forward it to the appropriate person for completion. WPS has recommended you hold off on changes to the bank account(s) until after the J8 transition in August, unless you are assured your changes will be completed by NGS prior to a June 30, 2012, deadline.
