The ISMA reminds you that you must renew your Indiana medical license by midnight on Thursday, June 30.

You should have received an email notice from the Indiana Professional Licensing Agency about your license/residency permit renewal and also your Controlled Substance Registration (CSR) renewal.

Failure to act by June 30 will result in a gap in licensure and late fees of $50 per license/registration.

If you do not have an email address, you should have received a paper form by mail.
